Eligible patients: We will alert you via email after your appointment when your scripts have been sent to your nominated pharmacy listed on your consent form.
Ineligible patients: If you are deemed ineligible at your appointment, you will get a full refund. Please keep in mind you are not eligible for a refund if you provide false or misleading information in your consent form.
We require all patients to have a recent general GP health summary on their file. If you have not already, please either obtain your health summary by calling your GP Clinic and then emailing it through to us at admin@newwaveclinic.com.au or allow consent below for us to obtain your health summary on your behalf.
Please put your most recent GP Clinic that you have been to. We do not accept previous Medicinal Cannabis clinics as a general GP clinic.
To avoid any delay with your appointment please make sure all details are correct on the form. If you are unsure of the clinics email address please contact the clinic directly to obtain this information.
